Important scope noteThis assessment analyzes your saddle’s design specifications based on serial number data and brand standards. It cannot account for unrecorded custom adjustments made during the original build or any unpublicized changes to model specifications over time.
Before you begin

• The intake form requires signing into a Google account.
  
  • If you are unable to log in to a Google account (this sometimes happens in other countries), please let me know and we will complete the form in another format of your choice.
    
• Your responses are automatically saved. You can return to a partially completed form using the same link.
  
• You may edit your responses after submitting, if needed.
  

What to expect after you submit

• Once you submit the form, I will be automatically notified.
  
• I’ll review your information and contact you by email if anything is missing or needs clarification.
  
• Your report will be delivered by email, typically within 24 hours.
